Social Worker

imageLicensed Independent Social Workers (LISW) and those with a Masters in Social Work (MSW), often assist patients with life transitions during their time utilizing Home Health Care. By analyzing areas of psychosocial need, Medical Social Workers are able to provide and connect patients and their families with relevant services to help improve their quality of life. These services are tailored to the individual and can be used for a variety of situations and conditions. Most often, Social Workers are called upon to offer assistance in areas of:

  • Therapy
  • Grief Counseling
  • Financial Assistance
  • Crisis Intervention
  • Social Connections
  • Advance Directives

Medical Social Workers typically work in conjunction with a team of physicians and nurses to appropriately arrange care, treatment, and aid recovery of their patients. Through education, Social Workers arrange for patients to successfully overcome many of the obstacles to a rich and rewarding life that may initially present during the discovery, treatment, and recovery phases of an illness or injury.
